This is not a procedure I would do acutely or if the patient wasn't having symptoms.  This had been swollen for over a month and she was having some discomfort from the inflamed area.  In cases such as these, sometimes the drainage of the fluid allows the area to start settling down, which is what happened in this case.  I should also comment on sterility in these procedures.  Sterile gloves were not available and so I generally wash my regular gloves in chlorhexidine before I start.  As well, the entire surface was cleansed 3 times, again, with chlorhexidine before I started taping.  Had this been an intraarticular drainage, I likely would have deferred until I could obtain sterile gloves.
