High-Intensity Focused Ultrasound (HIFU) has recently become one of the most advertised treatments for prostate cancer.  Here, PCRI's Alex asks Dr. Mark Scholz, a medical oncologist specializing exclusively in prostate cancer, whether or not HIFU lives up to the hype.

0:33 Is HIFU as effective as it's made out to be? HIFU is effective at destroying prostate tissue; however, the doctors that perform HIFU are urologists who generally do not specialize in targeting like radiation oncologists and interventional radiologists do.  This problem is compounded by the fact that HIFU is only really attractive as a focal therapy (or possibly for salvage) which requires even more precise targeting.  

3:34 What kind of questions should a patient be asking their urologist before undergoing HIFU? It might be helpful to ask how many procedures they've done and to check online reviews.

4:05 What side effects usually accompany HIFU treatment? Erectile dysfunction is the most common side effect, but there is a small risk of developing a fistula, although this is rare with a skilled doctor.

4:37  Do you know of any practitioners that you trust to perform HIFU without the risk of side effects? Dr. Mark Scholz does not currently know if anyone he trusts to be an expert at performing HIFU

4:55 Do you know of any cases in which HIFU has failed?  Dr. Scholz has seen cases of every sort of focal treatment that has failed because it missed the target.
